Team Guide Ads 2017 season

Now is the time to place your ad for the 2017 PTHS Soccer Team Guide! Ordering is easy!

  • You can create your own ad in Word, PowerPoint, or other image editing software, save as a PDF, and email it to the address on the order form.
  • If you would like your ad created for you, select the need design option. You’ll provide the text and photo and we’ll do the rest. A proof of your ad will be sent back to you for review.
  • Personal & Corporate ads can be emailed, with a check (payable to PTHS Soccer Boosters) mailed to Nancy Gill, 100 Windermere Court, McMurray, PA 15317. Please view the order form for details.

Show your team spirt by ordering apparel with the both boys’ and girls’ logos!

  • Please complete the forms and mail or drop off check before the deadline.

Reimbursement for Booster expenses 2018 calendar year

Budgeted Booster expenses are reimbursable through submission of the Reimbursement form, which must have a Board member or Committee Chairperson approval signature.

2018 REQUIREMENTS

To participate in practices, every athlete must have completed the following requirements:

FamilyID Account

FamilyID is a secure registration platform used to register your PTHS athlete. Once you set up a Family ID account you can login to update the information each year.

2/  

Physical & Waiver forms

  • Physicals must be completed on or after June 1, 2018.
  • Follow the steps on FamilyID to register your player by Aug 3.
  • Turn in Sections 5 & 6 of the 2018-19 PIAA CIPPE Forms & the Athletic Fee form (available soon)
  • Forms and payment (check payable to PTSD) must be turned in to the Athletic Office by 8/3.

Baseline Testing

  • All players must have a baseline test (concussion baseline) on file with our trainer. Testing must be done every two years. If your child’s current baseline expires prior to the completion of their season they will need to schedule a new baseline for this summer.
